All medical services offered by C & S Nurse Concierge require evaluation and approval by a licensed medical provider through a Good Faith Exam (GFE).

Good Faith Exam Fee

1. $50 for patients who have not completed a current GFE.
2. Valid for 12 months from the date of completion.
3. Included at no additional charge with the Initial Medical Weight Loss Consultation.

Treatment Eligibility

Completion of a Good Faith Exam does not guarantee treatment approval. Final treatment recommendations are determined by the provider.

Appointment Cancellations

We require a minimum of 24 hours notice for appointment cancellations or rescheduling.

No Show Policy

Failure to attend a scheduled appointment without notice may result in forfeiture of deposits or future scheduling restrictions.

Medication Policy

Due to the nature of compounded medications and prescription products:

1. No refunds
2. No exchanges
3. No returns

once medications have been ordered, prepared, dispensed, or shipped.

Laboratory Testing

Laboratory services are non-refundable once specimens have been collected or submitted for testing.

Memberships & Packaging

Promotional packages and memberships are non-transferable and may have expiration dates as stated at purchase.
